Personalized 1-1 Virtual Consultation (45 min)
We offer one-on-one virtual consultations with seniors and their support systems (if applicable) to explore personalized needs and preferences. Our tailored approach ensures that seniors are seen, heard, valued, and empowered. We take the time to deeply understand each unique situation, offering guidance that aligns with their lifestyle and goals. Note: Requests for in-person consultation sessions may be considered, however travel charges may apply.
Contracted Services
The following services can be purchased individually or bundled for a discounted rate after the Personalized 1-1 Virtual Consultation.
- Housing Navigation Services: We provide expert guidance on finding the right senior living arrangements, from independent living to continuing care communities. Our team works closely with seniors and their support network to research, evaluate, and explore suitable housing options, simplifying the decision-making process.
- Service Liaison: We can act as a liaison between seniors, their support systems, and service providers. Our goal is to ensure that seniors receive the best care & service; while ensuring communication among the partners is healthy and timely decisions are made by the right people throughout the process.
Partners in Care & Service (PICS) Virtual Hub
Recognizing the importance of connection and empowerment in making informed decisions, Nana’s Navigator offers the Partners in Care & Service (PICS) Hub. This virtual hub provides a space for seniors, their support network, and service providers to stay connected, collaborate, access resources, and stay informed.
